Lilith and Inarius gathered like-minded followers and retreated to Sanctuary for a break from the Eternal Conflict, and eventually, the demon and the rogue angel had Rathma, a first-generation nephalem, and the first child to be conceived between two such beings. Another nephalem came along, however, the rogue angels and demons started to be worried about their new offspring's increasing strength and were frightened by this new breed of beings. Some wanted to destroy them but Lilith is afraid for her children and believing that they were the way to end the conflict for good was determined to kill every follower of Inarius who wanted to kill them.

Inarius was shocked by Lilith's deceit and cruelty, however, he could not decide to kill her but he did send her back to the Void. He also ensured that every subsequent generation of nephalem became weaker which avoided the threat of renegade demons as well as angels feared of their progeny that would rise up against them. However, Lilith was honored in mythology, referred to as the First Mother by a few and encouraging cults to revere her. Lilith did make a return to Sanctuary prior to this and altered potential of Worldstone to grant nephalem abilities once more. Although Lilith has been able to create chaos and return temporarily to Sanctuary in various guises, Inarius was successfully able return her to the Void.

As shown in the official trailer for Diablo 4. a member of the Triune was able to trick two adventurers and one scholar into entering a ritual chamber to ensure that Lilith could be finally released many thousands of years after her initial banishment from Sanctuary. Lilith's return has sparked a new generation of worshipers and cultists throughout the world of mortals since her return was viewed by some as the answer to ending an Eternal Conflict. Lilith is arouses the most naive desires within the souls of all who meet her. her return to Sanctuary is bound to result in a return to darkness and despair , too. Her final goal - to make sure that the nephalem rises and rise again - will likely be the central theme of Diablo 4's plot.

The developers have promised a much sensible and more solid structure for Diablo 4's story than the predecessor Diablo 3. Even though Diablo 3's Pandemonium Event in Diablo 2 is not a must-play however, it may provide clues on how Lilith is going to carry out her plan of domination and may hint that she may try to summon Diablo or other Primordial Evils. If Lilith is seeking to empower the nephalem, it could provide interesting antagonists or even help to strengthen the player's personal character as the game progresses as well.
